2022-23 FIU MEN'S BASKETBALL TRYOUT INFORMATION

Students must fill out and turn in all the paperwork prior to the tryout, and forms can be turned on Tuesday, October 4. Paperwork can be signed in the athletic training room with
Dr. Michael Walker, in the SAAC building with
Shirley Caballero, or in the compliance office in the Ocean Bank Convocation Center with
Jimmy Fleming.
SAAC (Room 150)
You must be a full-time student enrolled in at least 12 credit hours at FIU at the time of tryouts, and must currently hold and maintain a 2.4 cumulative GPA.
Shirley Caballero located in the Student-Athlete Academic Center (SAAC), must verify and sign off on this information

Athletic Training Room (Ocean Bank Convocation Center, Room 156)
Bring a COMPLETED physical packet and the following forms to
Dr. Michael Walker : Athletic Tryout Release, Parent Information, Insurance Verification, Physical Form, Health History Questionnaire, Orthopedic Questionnaire, Training Room Information, & Sickle Cell Trait Information.
The internal physical MUST be performed by a medical doctor and must have taken place during the last six months. Make sure the physical is signed and stamped by the doctor. Appointments can be made at Student Health and Wellness Center or with your primary care physician (be sure to attach a business card). Schedule this appointment as soon as possible so that you can complete it PRIOR to Oct. 5.
** All potential walk-ons must have a sickle cell test completed prior to October 5 **
- If you have never been sickled cell tested in the past, you can schedule an appointment at the Student Health Center to be completed.
- If you have been tested for sickle cell, you must retrieve a copy of your status stating if are you either positive or negative.
FIU Athletic Compliance (Ocean Bank Convocation Center, Room 154)
Bring a printed class schedule and Panther ID showing proof of enrollment of a minimum of 12 credit hours to
Jimmy Fleming in the FIU athletic compliance office to get a signature verifying full-time status. If you participate in a team tryout and are added to the official roster, you will be required to register with the NCAA Eligibility Center.
